The Teva-Dasatinib Patient Care ProgramSM provides reimbursement and financial assistance for enroled patients. Download these program materials designed to quickly enrol patients into the program and provide details on the financial assistance that is available. For patients who qualify, the pharmacy will be reimbursed in the same way as any other third-party plan.
How to enrol patients:
Enroling a new patient into the Teva-Dasatinib Patient Care ProgramSM is easy.
- Fax or email the completed enrolment form: Prescribers, pharmacists, technicians and pharmacy assistants can gather the patient’s information and send the completed enrolment form by fax to 1-877-211-5159 or email to info@DasatinibTevaCanada.com.
- Pharmacy enrolment portal: Additionally, pharmacists, technicians and pharmacy assistants can also complete patient enrolment through the Pharmacy Enrolment Portal.
- Patient self-enrolment: Patients also have the option of enrolling themselves with the help of a Bilingual Patient Care Specialist by calling 1-877-211-5156 (Monday to Friday, 8 am – 8pm).
